Who files this form

Use 2025 Schedule OR-DONATE if you want to donate some or all of your Oregon income tax refund to the charities listed on the schedule. You must include the schedule with your Oregon income tax return to have your refund applied to those donations.

What you'll need

The schedule asks for your last name, Social Security number (SSN), and donation amounts for your chosen charities.

Include the completed schedule with your Oregon income tax return. Keep a copy of Schedule OR-DONATE, or the adjustment notice if the donation amount is adjusted, with your records as your receipt.

How to file

  1. Enter the amounts you want to donate next to the listed charities.
  2. Add lines 1 through 29 and enter the total on line 30.
  3. Enter that total on your 2025 Oregon return: Form OR-40, line 49; Form OR-40-N, line 74; or Form OR-40-P, line 73.
  4. Include Schedule OR-DONATE with your Oregon income tax return.

Common mistakes

  • Do not leave out the schedule. It must be included with your Oregon income tax return for your refund to be applied to donations.
  • Use UPPERCASE letters and blue or black ink.
  • Print at actual size (100%). Do not submit photocopies or use staples.
  • Your donation total cannot reduce your net refund below zero. If your refund is used to pay a debt or your chosen donations exceed your available refund, the donations will be prorated based on the available refund.
  • Keep a copy of the schedule or adjustment notice as your receipt.
