What you'll need

Have these details ready to complete the voucher and payment:

  • The estate’s or trust’s nine-digit FEIN. If an estate has not received its FEIN, use the decedent’s SSN. If a trust has not received its FEIN, use nine zeroes. Follow the number with “F” and omit hyphens.

  • The payment amount and, for fiscal filers, the fiscal beginning and ending dates.
  • A check or money order for a mailed payment, or bank account information for an electronic funds transfer through myPATH.

Common mistakes

  • Using the voucher for another purpose. The 2025 PA-41 V is only for payment of tax owed with a 2025 PA-41 return.
  • Paying $15,000 or more by a nonelectronic method. These payments must be electronic. Failure to meet this requirement carries a penalty of 3% of the payment amount, up to $500.
  • Leaving the FEIN off the check or money order. The department needs it to apply the payment accurately if the payment becomes separated from the voucher.
  • Entering the wrong payment amount. The form specifically asks you to enter the amount carefully.
