Who files this form
Use the 2025 Schedule OR-ADD-DEP if you have more than three dependents. Enter information for each additional dependent who is not listed on the second page of your Oregon income tax return.
If you have more than eight dependents, fill out and include another Schedule OR-ADD-DEP.
