Oklahoma · Oklahoma Tax Commission · Used for tax years 2025

Oklahoma Form EF-V is a business filers income tax payment voucher for 2025. It identifies the entity and lists the balance due and payment amount to accompany a check or money order after the return has been filed.

Who files this form

Form EF-V is for business filers paying a balance due on a 2025 Oklahoma Form 512, 512-E, 512-S, 513, 513-NR or 514 after filing their return electronically or on paper.

Using this voucher is strongly encouraged, but there is no penalty for not using it. Individual filers completing Form 511 or 511-NR should use Form 511-V instead.

  • One check or money order payable to the Oklahoma Tax Commission, with the entity's name on it.

Your income tax return should already be filed. Do not enclose a copy with the voucher.

  • Do not fold the voucher or tear or cut below the indicated line.
