EF-V: 2024 Form EF-V Business Filers Income Tax Payment Voucher (2024)

Oklahoma · Oklahoma Tax Commission · Used for tax years 2024

Oklahoma Form EF-V is a business income tax payment voucher for a balance due on a 2024 Form 512, 512-S, 513, 513-NR or 514 that has already been filed. It identifies the entity and lists the balance due and payment amount.

Who files this form

Form EF-V is for business filers paying a balance due on a 2024 Oklahoma Form 512, 512-S, 513, 513-NR or 514 after the return has already been filed, either electronically or on paper.

Using the voucher is strongly encouraged, but there is no penalty for not using it. Individual filers completing Form 511 or 511-NR should use Form 511-V instead.

What you'll need

For a payment by mail, prepare:

  • The 2024 Form EF-V voucher, with the entity name, address, daytime phone number, Federal Employer Identification Number, tax period, balance due and payment amount.
  • One check or money order per voucher, payable to the “Oklahoma Tax Commission.” Make sure the entity name appears on the payment.

Do not include a copy of your Oklahoma income tax return. The return should already be filed.

How to file

After filing your 2024 income tax return electronically or on paper:

  1. Complete Form EF-V and cut along the dotted line to detach the bottom voucher.
  2. Prepare one check or money order payable to the “Oklahoma Tax Commission,” with the entity name on it. Do not send cash.
  3. Put the voucher and payment loose in the envelope. Do not staple or otherwise attach them, and do not include a copy of the return.

Common mistakes

  • Sending cash. Use a check or money order for a mailed payment.
  • Sending more than one check or money order per voucher.
  • Leaving the entity name off the check or money order.
  • Including a copy of the income tax return. The return should already be filed separately.
  • Folding, stapling or paper-clipping the voucher, or attaching the payment to it. Place the voucher and payment loose in the envelope.
  • Tearing or cutting below the marked line.
  • Using EF-V for an individual Form 511 or 511-NR. Those filers should use Form 511-V.
