Who files this form
Form 591-F is required when the person applying for the Parental Choice Tax Credit (PCTC) is not the parent or legal guardian of the student(s) listed in the application. Both the applicant and the parent or legal guardian must complete and sign the form.
A parent or legal guardian applying for their own student(s) does not need this form. A new consent form is required each year a non-parent or non-guardian applicant applies. This form identifies the school year as 2025-2026.

Common mistakes
- Mailing the form to the Oklahoma Tax Commission. The form says not to mail it. Upload it with the PCTC application on OkTAP.
- Leaving one participant’s part incomplete. Both the parent or legal guardian and the applicant must fully complete and sign the form.
- Reusing a prior year’s consent or leaving it off an application. A consent form must be completed each year and uploaded with each PCTC application for the listed student(s).
- Allowing another applicant to apply for the same student(s). The consent states that no other applicant may apply on their behalf for the applicable year.
- Providing false or incomplete information, or failing to report changes. The form warns of potential legal consequences for falsified, misrepresented, or omitted information. Participants must promptly notify the OTC at parentalchoice@tax.ok.gov if information becomes inaccurate or incomplete.
