IT 1041: 2024 Fiduciary Income Tax Return (2024)

Ohio · Ohio Department of Taxation · Used for tax years 2024

The 2024 Ohio IT 1041 is the Fiduciary Income Tax Return for trusts and estates. It reports taxable income, Ohio income adjustments, credits, payments, and any tax due or refund.

What you'll need

Include page 1 of the federal Form 1041. Ohio IT 1041 starts with federal taxable income from federal Form 1041, line 23.

Gather the trust or estate’s FEIN, the decedent’s SSN for an estate, fiduciary contact information, and beneficiary names, addresses, tax identification numbers, and amounts distributed.

Include these documents when applicable:

  • Forms 1099 and W-2 for the net payment worksheet.
  • Ohio IT/SD 2210 for the estimated-tax underpayment penalty.
  • Documentation or certificates for refundable business credits. Schedule XII requires tax credit certificates and/or Ohio IT K-1s to verify each credit claimed.
  • Documentation for electing small business trust (ESBT) income or losses not shown in federal taxable income.
  • Copies of the lump sum retirement or lump sum distribution credit worksheets, and the work-based learning credit certificate, for those credits.
  • Ohio IT RCTE for the estate or trust resident credit, or Ohio IT NRCE for the estate nonresident credit.
  • A letter of exemption if checking the “qualifying pre-income tax trust” box.

How to file

Complete the return using black ink, UPPERCASE letters, and whole dollars. Fill in the reporting period, identification details, and applicable trust or estate selections.

The fiduciary or trust officer must sign the return and provide their title, date, and phone number. Place supporting documents, including Ohio IT K-1s, after the last page. Do not staple or paper clip.

Mail the return to:

Ohio Department of Taxation

P.O. Box 2619

Columbus, OH 43216-2619

If paying by check, make it payable to Ohio Treasurer of State, include Ohio IT 1041 UPC, and write the FEIN on the check.

Common mistakes

  • Do not staple or paper clip the return or supporting documents.
  • Use only black ink, UPPERCASE letters, and whole dollars. Leave department-use-only areas blank.
  • Do not omit the required fiduciary or trust officer signature.
  • Put a minus sign in the provided box when a line amount is negative, unless that line directs you to enter zero.
  • Estates should skip from line 3 to line 8.
  • On an amended return, enter zero on line 18 for the credit carryforward.
  • Include information for all beneficiaries, both resident and nonresident. Use an additional sheet if needed.
  • Carry Schedule X apportionment ratios to six decimal places.
  • Include the tax credit certificates and/or Ohio IT K-1s needed to verify refundable credits.
