ND-1PRV: Form ND-1PRV - Paper Return Voucher (2025)
North Dakota · ND Tax Commissioner · Used for tax years 2025
Form ND-1PRV is North Dakota's 2025 individual paper return payment voucher. It identifies the taxpayer and payment amount and accompanies a paper return and payment by check or money order.
Who files this form
Use the 2025 Form ND-1PRV if all three apply:
- You are filing your 2025 Form ND-EZ or Form ND-1 on paper.
- Your return has a balance due.
- You are paying that balance by paper check or money order.
Do not use this voucher if you file your return electronically, pay electronically, or make an extension payment.
When it's due
For tax year 2025, the balance due on Form ND-EZ or Form ND-1 must be paid on or before April 15, 2026, to avoid late payment penalties and interest.
What you'll need
Have these ready:
- Your paper 2025 Form ND-EZ or Form ND-1 return.
- A check or money order for the balance due.
- Your name, mailing address, Social Security number, and payment amount to complete the voucher. The voucher also has fields for your spouse’s name and Social Security number.
How to file
- Make your check or money order payable to ND State Tax Commissioner.
- Write the last four digits of your Social Security number and “2025 ND-1PRV” on the check or money order.
- Complete the voucher and detach it from the page.
- Mail the voucher, payment, and paper return to:
Office of State Tax Commissioner
PO Box 5621
Bismarck, ND 58506-5621
Only checks drawn on a U.S. or Canadian bank, in U.S. dollars, and using a standard 9-digit routing number are accepted.
Instead of mailing a payment, you may pay online by electronic check, debit card, or credit card. Go to www.tax.nd.gov and select “Make A Payment.” Do not use this voucher if paying electronically or filing your return electronically.
Common mistakes
- Using this voucher when filing the return electronically or paying electronically. The form says not to use it in either case.
- Using this voucher for an extension payment. Use Form ND-1EXT instead if you have an extension to file Form ND-1 and want to make an advance payment of the expected balance due.
- Forgetting to write the last four digits of your Social Security number and “2025 ND-1PRV” on the check or money order.
- Sending a check that does not meet the bank, currency, and routing-number requirements printed on the form.
- Leaving out required identification information. The form warns that failure to provide the required Social Security number or FEIN may delay or prevent processing.
