What you'll need
Have the corporation’s identifying information ready, including its legal name, address, federal employer ID number, NAICS code, and Secretary of State ID. The return also asks for income, deductions, assets, liabilities, and other financial information.
Include these documents when applicable:
- Federal income tax return: Attach a copy, or complete Schedule G if you do not attach it.
- Form CD-425: Required if claiming a tax credit on Schedule A, Line 4, or Schedule B, Line 24e.

Common mistakes
- Handwriting information: The web fill-in guidelines say not to handwrite any information.
- Using commas or brackets in amounts: Do not enter commas. Use a minus sign, not brackets, for negative numbers.
- Using the wrong print settings: Do not print on both sides. Set page scaling to “none” and uncheck Auto-Rotate and Center.
- Sending photocopies or mixing form types: Submit original returns only and do not mix form types.
- Omitting required attachments: Form CD-425 must accompany the specified tax credit claims, and a copy of Form D-403 NC K-1 must accompany a partnership payment claimed on Line 24c.
- Rounding the apportionment factor incorrectly: Calculate it to four places to the right of the decimal.
- Missing the indicator circles: Fill in the applicable circles for negative amounts and overpayments where the form directs.
