Common mistakes

  • Leaving line A blank. All filers must complete it.
  • Using one form for multiple certificates. The form requires a separate IT-665 for each certificate issued by Empire State Development.
  • Omitting the certificate copy. Submit a copy with your return.
  • Skipping Schedule C as a fiduciary. Fiduciaries must also complete Schedule C under either line A answer.
  • Leaving out an intermediary entity’s details. If you marked No on line A and received your share from an entity other than the one on the certificate, provide that entity’s name, identifying number, and type.
