IT-258: Claim for Nursing Home Assessment Credit

Form IT-258 claims New York’s nursing home assessment credit against personal income tax. It reports the nursing home’s information and the 6% base-rate portion of the assessment you paid directly, not your total care expenses. Submit it with Form IT-201 or Form IT-203.
