IT-213-ATT: Child Information for Empire State Child Credit

Form IT-213-ATT is an attachment to Form IT-213 for reporting additional qualifying children for the Empire State Child Credit. It lists the name, Social Security number or individual taxpayer identification number, and date of birth for each qualifying child not included on Form
