Common mistakes
- Using an incorrect or incomplete SSN on the voucher. The full number must match your Social Security card and your New York State income tax return. A taxpayer ID number must match your return. A mismatch may prevent proper crediting of your payment.
- Using a different name from your return. Check the spelling and enter your first name, middle initial, and last name in the spaces provided.
- Entering only the total payment. Enter the New York State, New York City, Yonkers, and MCTMT amounts separately before entering the total. Leave lines with no amount blank.
- Formatting a foreign address incorrectly. In the city box, enter the city, province or state, then country. Do not abbreviate the country name. Follow that country's postal code format.
- Attaching the payment to the voucher. Do not staple or clip it. Detach any check stubs before mailing.
