New York · NY DTF · Used for tax years 2024
Form IT-2102.6, Certificate of Income Tax Withheld, reports your total wages before deductions and New York State, New York City, and Yonkers income tax withheld by an employer. It also records why you could not obtain a federal Form W-2 from that employer.
Who files this form
Use Form IT-2102.6 only when every effort to obtain a federal Form W-2, Wage and Tax Statement, from your employer has failed. Submit the certificate with your tax return for that employer.
If you cannot obtain a W-2 from more than one employer, complete a separate certificate for each employer.
