What you'll need

Have the following information ready:

  • The trust or estate’s name as shown on federal Form SS-4, and its employer identification number.
  • The fiduciary’s name, title, and address.
  • The total estimated tax payments being allocated and each beneficiary’s share.
  • Each beneficiary’s name, address, and Social Security number or employer identification number.
  • For a beneficiary who may file jointly, the spouse’s Social Security number is optional.

If you allocate payments to more than 16 beneficiaries, use an additional Form IT-205-T or a separate sheet. A separate sheet must follow the form’s format and include your name and taxpayer identification number.

How to file

If you make the election with Form IT-205, Fiduciary Income Tax Return, submit Form IT-205-T with that return. Otherwise, file Form IT-205-T separately.
