Who files this form

CT-60 is an affiliated entity information schedule attached to Form CT-3, CT-3-A, or CT-3-S.

The schedule collects information about qualified subchapter S subsidiaries (QSSSs), shareholders, and other related entities. It includes questions about corporate ownership or control, affiliated federal groups, partnerships, single-member LLCs (SMLLCs), and tax-exempt DISCs.

The form does not provide a complete rule explaining which corporations must file it.

What you'll need

Have the information needed for the parts that apply to your corporation:

  • The corporation’s legal name, employer identification number (EIN), and period ended.
  • QSSS names and addresses, federal election effective dates, and federal EINs or temporary filing numbers for each QSSS and its parent.
  • Shareholder names, addresses, and EINs or Social Security numbers, plus distributions reported on federal Form 1120S, Schedule K, line 16d and/or 17c.

  • In Schedule B, line 2, mark all applicable entity types whose activities affect your federal return, and attach federal Form 851 if applicable.
  • If you filed a consolidated federal return, mark the box on line 4 and complete lines 4a through 4d.
  • Remember to attach CT-60 to Form CT-3, CT-3-A, or CT-3-S.
