MO-1040V: Individual Income Tax Payment Voucher (2025)

Missouri · Missouri DOR · Used for tax years 2025

Form MO-1040V is Missouri’s 2025 Individual Income Tax Payment Voucher for payments sent separately from your income tax return. It identifies you and the amount you are paying so the payment can be processed efficiently and accurately.

Who files this form

Use Form MO-1040V to send a payment separately from your Missouri income tax return if you:

  • Filed electronically but did not pay electronically, or
  • Did not pay in full when you filed your return.

Do not use this voucher for extension payments. Use Form MO-60 or visit the Department of Revenue website to pay online.

When it's due

For tax year 2025, your income tax return and payment are due no later than April 15, 2026.

You may file your completed return and send your payment later, but full payment must be submitted by April 15, 2026, to avoid interest and additions to tax for failure to pay.

What you'll need

To complete the voucher and payment, have:

  • Form MO-1040V for 2025.
  • Your name, address, and Social Security number (SSN). For a combined return, also have your spouse’s name and SSN, in the order shown on the return.
  • Your payment amount in whole dollars.
  • A check or money order in U.S. funds.
  • Your daytime telephone number to write on the check or money order.

Do not include a copy of your previously filed return.

How to file

  1. Complete the name and address block and SSN fields. For a combined return, enter both spouses’ information in the order shown on the return, using the designated spouse fields.
  2. Enter your name control, using the first four letters of your last name and the examples printed on the form. Complete your spouse’s name control if filing a combined return.
  3. Enter the payment amount in whole dollars. Print in capital letters using black or dark blue ink.
  4. Make your check or money order payable to Missouri Department of Revenue. Write your name, address, SSN, daytime telephone number, and “2025 MO Income Tax” on it.
  5. Detach the voucher at the perforation and mail it with your payment to:

Missouri Department of Revenue

P.O. Box 371

Jefferson City, MO 65105-0371

Do not mail the voucher if you have already submitted payment. Do not include a copy of your previously filed return.

Common mistakes

  • Using the voucher for an extension payment. Use Form MO-60 or visit the Department’s website to pay online instead.
  • Mailing the voucher after already submitting payment. The form says not to mail it in that situation.
  • Sending cash or postdating a check. Do not send cash. Checks will be cashed upon receipt, so do not postdate them.
  • Including a previously filed return. Mail only the voucher and payment, not a copy of that return.
  • Using red ink or pencil. Print in capital letters with black or dark blue ink.
  • Paying late. Full payment for tax year 2025 must be submitted by April 15, 2026, to avoid interest and additions to tax for failure to pay.
