MO-1040V: Individual Income Tax Payment Voucher (2024)

Missouri · Missouri DOR · Used for tax years 2024

Form MO-1040V is Missouri’s 2024 Individual Income Tax Payment Voucher, sent with a payment made separately from your income tax return. It identifies the taxpayer and payment amount so the payment can be processed efficiently and accurately.

Who files this form

Send Form MO-1040V with your payment if you owe money on an electronically filed return or did not pay in full when you filed your income tax return.

This voucher is for payments sent separately from your return. Do not use it for extension payments. Use Form MO-60 or visit the Department of Revenue website to pay online instead.

When it's due

For tax year 2024, your income tax return and payment are due no later than April 15, 2025.

You may file your completed return and send your payment later, but full payment must be submitted by April 15, 2025, to avoid interest and additions to tax for failure to pay.

What you'll need

To complete the voucher and prepare your payment, have:

  • Your name, address, and Social Security number (SSN).
  • Your spouse’s name and SSN if filing a combined return. Use the same order as on your return.
  • Your name control, which is the first four letters of your last name, and your spouse’s name control if filing a combined return.
  • Your payment amount in whole dollars.
  • A check or money order in U.S. funds and your daytime telephone number to write on it.

Do not include a copy of your previously filed return.

How to file

  1. Complete the voucher’s name and address fields, SSN fields, name control fields, and payment amount. Print in capital letters using black or dark blue ink.
  2. Make your check or money order payable to Missouri Department of Revenue.
  3. Write your name, address, SSN, daytime telephone number, and “2024 MO Income Tax” on the check or money order.
  4. Detach the voucher at the perforation and mail it with your payment to:

Missouri Department of Revenue

P.O. Box 371

Do not mail a copy of your previously filed return with the payment.

Common mistakes

  • Using this voucher for an extension payment. Use Form MO-60 or visit the Department of Revenue website to pay online instead.
  • Sending cash or a postdated check. Do not send cash. Checks are cashed upon receipt, so do not postdate them. Payments must be in U.S. funds.
  • Including a previously filed return. Mail only the voucher and payment, not a copy of that return.
  • Using red ink or pencil. Print in capital letters using black or dark blue ink.
  • Putting combined-return information in the wrong order. Enter your and your spouse’s SSNs and name controls in the order shown on the return, using the designated spouse fields.
  • Entering cents on the voucher. Enter the payment amount in whole dollars.
