Michigan · Michigan Treasury · Used for tax years 2025

Michigan Form 4906 is the 2025 amended return for insurance company corporate income and retaliatory taxes. It reports Michigan premiums, exemptions, credits, tax liability, payments, and any payment due, refund, or credit forward.

Who files this form

Form 4906 is Michigan’s amended return for insurance companies reporting Corporate Income and Retaliatory Taxes for tax year 2025.

Foreign and alien insurers complete the retaliatory tax calculation on lines 32 through 46. Domestic insurers enter the amount from line 31 directly on line 47.

What you'll need

Have the information needed to complete the return, including:

  • The company’s name, address, Federal Employer Identification Number (FEIN), and state of incorporation.
  • Michigan premium amounts, separated between qualified health insurance policies and all other policies, plus the premium deductions and disability insurance information requested on the form.
  • Amounts paid to the organizations listed on lines 24a through 24e, and Michigan examination fees or regulatory fees.
  • Prior-period credits, estimated payments, extension payments, withholding or Flow-Through Entity credit, payments made with or after the original return, and overpayments already received.

Where applicable, the form also calls for:

  • Form 5793, line 11, for tax liability after the Historic Preservation Credit.
  • Form 4902 for recapture of certain business tax credits.
  • Form 4899, line 38, for underpaid estimate penalty and interest.
  • A supporting document attached for the Workers’ Disability Supplemental Benefit (WDSB) Credit.
  • Detailed schedules for other fees on line 39 and other assessments on line 44.

How to file

Complete the company identification fields and the applicable calculations on both pages. The form includes a reason-code field for amending that refers to instructions, but those instructions are not provided here.

Continue to page 2 and sign the taxpayer certification. Provide the authorized signer’s name, date, title, and telephone number. The form also includes a preparer certification section.

The supplied form text does not give a mailing address, e-file directions, or instructions on where to send the return.

Common mistakes

  • Skipping referenced instructions. The form says to see the instructions before completing lines 5 through 23. Those instructions are not included in the supplied text.
  • Mixing up the retaliatory tax columns. Foreign and alien insurers use column A for burdens their state of incorporation would impose on a hypothetical Michigan company doing the same business there. Column B is for actual Michigan burdens on the taxpayer.
  • Leaving out required supporting material. Include detailed schedules for other fees and other assessments, and attach documentation for the WDSB Credit.
  • Missing the second-page signature. Page 1 specifically reminds filers to continue and sign on page 2.

Questions people ask

How are qualified health insurance policies reported?

Use column A for qualified health insurance policies and column B for all other policies. On line 22, multiply the adjusted tax base in column A by 0.9607% and column B by 1.25%.

Which payment period applies to the credits on line 24?

For the 2025 return, line 24 asks for amounts paid from January 1, 2024, through December 31, 2024, to the five organizations listed on lines 24a through 24e. This is a credit payment period, not a filing deadline.

Can an overpayment be carried forward?

Yes. Line 64 lets you designate an amount from line 63 to be credited forward as an estimate for the next tax year. Line 65 calculates the refund by subtracting line 64 from line 63.

Can I authorize Treasury to discuss the return with my preparer?

Yes. Check the authorization box in the taxpayer certification section.
