Common mistakes
- Leaving out the schedule. Form 4642 must be included with your MI-1040 so contributions are properly credited to the designated funds.
- Overlooking the effect of a donation. Contributions reduce your refund or increase your tax liability.
- Signing up again when already registered. If your Michigan driver’s license or state ID already has the donor heart symbol, you are already registered and do not need to complete the donor registry portion.
