Common mistakes

  • Leaving out the schedule: Include Form 4642 with your MI-1040 so your contributions are properly credited to the designated funds.
  • Overlooking the effect on your tax return: Contributions reduce your refund or increase your tax due.
  • Registering again unnecessarily: If your Michigan driver’s license or state ID already has the donor heart symbol, you are already registered and do not need to complete the donor registry portion.
  • Expecting Treasury to have your registry status: Treasury does not maintain a record of an individual’s registry status.
