What you'll need
The 2025 voucher asks for:
- Payment period end date in mm/dd/yyyy format.
- Your name and Social Security number.
- Your spouse’s name and Social Security number.
- Your street address, city or town, state, and ZIP code.
- Your phone number and email address.
- The amount enclosed.
If you mail your payment, include the voucher with a check or money order payable to Commonwealth of Massachusetts.

Or, complete and detach the voucher and return it with a check or money order payable to Commonwealth of Massachusetts. Fill in the name/address change indicator if your name or address has changed since 2024.
