355-7004: Corporate Extension Payment Worksheet and Voucher (2025)

Massachusetts · Massachusetts DOR · Used for tax years 2025

Massachusetts Form 355-7004 is a corporate extension payment worksheet and voucher. It calculates the tax due by subtracting advance and estimated payments from the estimated tax for the taxable year. An extension of time to file does not extend the deadline to pay the tax.

Who files this form

Form 355-7004 is for corporate excise taxpayers calculating and paying tax due for an extension.

The extension is automatic if, by the original return due date, you have paid the greater of:

  • 50% of the total tax ultimately due, or
  • The minimum corporate excise.

If worksheet line 3 shows no tax due, no further action is needed for the extension. If it shows tax due, that amount must be paid by the original return due date.

When it's due

For the 2025 tax year, the form lists these payment deadlines for calendar-year filers:

  • April 15, 2026: Business corporations and S corporations included in a combined group.
  • March 15, 2026: S corporations not included in a combined group.

An extension gives you more time to file the return, not more time to pay the tax.

What you'll need

Have the following information ready to complete the worksheet and payment details:

  • Estimated tax for the taxable year, which must be at least the minimum tax.
  • Advance or estimated payments already made, if any.
  • Tax period end date.
  • Business name, address and Federal Identification number.
  • Whether the business is incorporated in Massachusetts.
  • Payment amount.

The form does not list supporting documents to attach.

How to file

Calculate your payment using the worksheet:

  1. Enter the estimated tax for the taxable year on line 1. It must be at least the minimum tax.
  2. Enter advance or estimated payments on line 2.
  3. Subtract line 2 from line 1 to find the tax due on line 3.

Pay the full amount on line 3 by the original return due date. Pay online through MassTaxConnect at mass.gov/masstaxconnect.

Although the text includes voucher instructions, this copy is marked “For informational purposes only” and “This form must be filed electronically.” It also says taxpayers required to pay electronically must not use the paper voucher. See TIR 21-9 for electronic filing and payment requirements. No mailing address is provided.

Common mistakes

  • Treating an extension as extra time to pay. Interest applies to tax not paid by the original due date.
  • Missing the minimum payment needed for a valid extension. If you do not pay at least 50% of the tax due or the minimum tax, whichever is greater, by the original due date, the extension is void. Late-return and late-payment penalties and interest are then assessed from the original due date.
  • Estimating less than the minimum tax on line 1. The worksheet says the estimate must be at least the minimum tax.
  • Using the paper voucher when electronic payment is required. The form warns against this, and this copy is marked for electronic filing only.

Questions people ask

How much extra time does the extension provide?

Corporate excise taxpayers filing combined reports receive seven months. Other corporate excise taxpayers receive six months. Taxpayers filing unrelated business income tax returns receive eight months. The payment requirements must be met.

What if line 3 shows no tax due?

No further action is needed for the extension.

Will interest or penalties apply during an extension?

Interest is charged on tax not paid by the original due date. Tax not paid within the extended period is subject to a penalty of 1% per month, up to 25%, from the extended due date. If the extension is invalidated because payment requirements were not met, late-return and late-payment penalties and interest are assessed from the original due date.

Can Form 355U payments be allocated to subsidiaries?

Yes. The form says payments may be allocated to subsidiaries when Schedule CG is filed.
