Common mistakes

  • Using the wrong tax-period dates. For the 2025 form, calendar-year filers enter 01-01-2025 and 12-31-2025. Fiscal-year filers enter their appropriate dates.
  • Leaving loss boxes unmarked. The form directs you to mark an X in the box for a loss.
  • Omitting required statements. Lines 13, 14, 19, and 21 each say to enclose a statement.
  • Double-counting a gain or loss on line 11. Line 11 is for the specified Massachusetts long-term gain or loss from U.S. Form 4797, Part II, not included on line 10.
  • Transferring line 24 to the wrong place. A grantor or beneficiary enters this amount on Form 1, line 38c, or Form 1-NR/PY, line 42c.
