What you'll need

Enclose a copy of the federal return, including all schedules and statements, and copies of Schedules K-1. Enter the number of Schedules K-1 enclosed on page 1.

Depending on the entries on your return, also include:

  • Schedule P if the pension income exclusion is more than $31,110.
  • A computation for the federal estate tax deduction.
  • Schedule ITC and supporting documents for nonrefundable credits.
  • Forms W-2 or 1099 for withholding reported on line 20(b).
  • Form PTE-WH and/or Form PTET-CR for amounts reported on line 20(c).
  • Supporting schedules for Schedule M entries where requested.
  • The expense-allocation computation if you answer “Yes” to additional information question 2.
  • Federal Schedule J (Form 1041) if you made an accumulation distribution as described in additional information question 5.

Common mistakes

  • Leaving out required copies: Enclose the federal return with all schedules and statements, plus copies of Schedules K-1.
  • Completing Schedule A when it does not apply: Do not complete it for a simple trust or pooled income fund. Complete it only if you made additions or subtractions on page 1, lines 2 or 6, and claimed a charitable deduction on federal Form 1041.
  • Attaching a Revenue Agent’s Report to this return: If a federal audit changed taxable income reported for a prior year, submit a copy to the Department of Revenue, but do not attach it to Form 741.
  • Not identifying an amended return: Check the amended-return box on page 1 and explain the changes. Enclose a separate page if necessary.
