Who files this form

Complete Form 741-V if you owe tax on your electronically filed Kentucky fiduciary income tax return. Use the voucher when mailing your payment.

When it's due

For tax year 2024, payments should be postmarked on or before April 15, 2025, to avoid penalties and interest.

The Department of Revenue does not issue statements of liability before the April 15 payment deadline.

What you'll need

To prepare the voucher and mailed payment, have:

  • Your Form 741 amounts: additional tax due from line 21 and interest and penalties from line 23.
  • The estate or trust name exactly as listed on the income tax return.
  • The Federal Employer Identification Number and mailing address.
  • A check or money order payable to Kentucky State Treasurer.

How to file

For a mailed payment:

  1. Enter the estate or trust name, Federal Employer Identification Number, and mailing address on Form 741-V.
  2. Enter Form 741, line 21, under “Additional Tax Due” and line 23 under “Interest and/or Penalties.” Fill in the total payment.
  3. Make your check or money order payable to Kentucky State Treasurer. Write your name, address, Federal Employer Identification Number, and “2024 Form 741” on it.

Common mistakes

  • Sending cash. The form says not to send cash.
  • Attaching the payment to the voucher. Mail them together, but do not attach them.
  • Including a copy of the electronically filed return. Do not include it with Form 741-V and payment.
  • Using a different estate or trust name. The voucher name should match the income tax return.
  • Leaving identifying information off the payment. Write your name, address, Federal Employer Identification Number, and “2024 Form 741” on the check or money order.
  • Waiting for a statement before paying. The Department of Revenue does not issue statements of liability before the payment deadline. For tax year 2024, payments should be postmarked by April 15, 2025, to avoid penalties and interest.
