Who files this form
Use Form 740-V, Kentucky Individual Payment Voucher, if you owe tax on your electronically filed individual income tax return and are mailing your payment.
When it's due
For tax year 2024, payments should be postmarked on or before April 15, 2025, to avoid penalties and interest.
The Department of Revenue does not issue statements of liability before the April 15 payment deadline.

To prepare your 2024 voucher and mailed payment, have:
- Your Form 740 or Form 740-NP amounts: additional tax due from line 33 and interest and penalties from line 35.
- Your name and mailing address, with your name matching your income tax return.
- Your Social Security number and, if married filing jointly or on a combined return, your spouse’s Social Security number.
- A check or money order payable to Kentucky State Treasurer.
Do not include a copy of your electronically filed return with the voucher and payment.
