What you'll need
Have these items ready for the 2025 return:
- Federal Form 1120 with all supporting schedules and statements. The form requires you to include them.
- The designated filer's Kentucky Corporation/LLET account number, FEIN, address, incorporation information, and Kentucky business activity information.
- Information about group members, including their names, FEINs, ownership, mergers, departures, and exclusions, for Schedule U2.
- Completed Schedules U1 through U5, which the supplied form pages say to attach. Schedule U5 calls for Sections A through D for each group member.
- Schedule U9 totals, which are used to complete the LLET and income tax summaries on Form 720U.
- Officer information. Attach a schedule with the designated filer's vice president, secretary, and treasurer names, home addresses, and Social Security numbers. Schedule U1 also asks for the president's information.
Depending on your situation, additional attachments include Schedule DE for disregarded entities, Schedule U6 for specified pass-through entity interests, and an explanation of changes for an amended return.

Common mistakes
- Leaving the account number blank or using only six digits. The Kentucky Corporation/LLET account number is required and must contain nine digits. Add leading zeros if it has six digits.
- Skipping Schedule U1 questions. Questions 3 through 8 must be completed for every corporation in the combined group. Questions 1 and 2 must also be answered for an initial return or when the corporation did not file or was not included in the prior year's combined group under the same name and FEIN. The form warns that missing answers may result in a request for a delinquent return.
- Omitting federal attachments. Include federal Form 1120 and all supporting schedules and statements.
- Omitting explanations. Attach an explanation of changes for an amended return. Complete Box G for a final or short-period return.
