K-40V: Kansas Payment Voucher (2025)

Kansas · Kansas DOR · Used for tax years 2025

Form K-40V is the 2025 Kansas Individual Income Tax Payment Voucher. It identifies the taxpayer and payment amount and includes boxes for amended and extension payments.

Who files this form

K-40V is the 2025 Kansas Individual Income Tax Payment Voucher for sending a tax payment by check or money order.

The printed directions tell you to mail the voucher with your payment if you already mailed your return, or if you filed electronically but did not pay electronically.

When it's due

For tax year 2025, pay your tax due on or before April 15, 2026. Payments not made by that date are subject to penalty and interest.

An extension gives you more time to file, not more time to pay. The form also notes that when a due date falls on a Saturday, Sunday, or legal holiday, returns and payments are due the next regular work day.

What you'll need

Have the following ready to complete the voucher:

  • Your name, mailing address, Social Security number, and the first four letters of your last name.
  • The same information for your spouse if filing a joint return.
  • Your daytime phone number and payment amount.
  • A check or money order for the full tax due, payable to Kansas Income Tax, with your Social Security number printed on it. The voucher says to write your SSN(s) on the payment.

How to file

Complete the voucher in black ink. Print the first four letters of each applicable last name in uppercase. Mark the name or address change box if that information changed since last year. Mark the extension payment box if filing an extension.

If sending payment with your return, place the voucher and payment loosely in the envelope. Do not attach them to the return or to each other.

The form prints two different mailing addresses:

  • The directions say that if you already mailed your return, or filed electronically without paying electronically, mail the payment and voucher to: Kansas Income Tax, Kansas Department of Revenue, PO Box 750260, Topeka, KS 66699-0260.
  • The voucher itself says to mail to: Kansas Department of Revenue, PO Box 3506, Topeka, KS 66625-3506.

The text does not explain this difference. For voucher assistance, contact the department at 785-368-8222.

To pay electronically, visit www.kansas.gov/payment-portal/ or go to ksrevenue.gov and log in to the Kansas Customer Service Center.

Common mistakes

  • Using ink other than black to complete the voucher.
  • Submitting a photocopy of the form. The voucher says not to submit photocopies.
  • Forgetting to put your Social Security number(s) on the check or money order.
  • Attaching the voucher or payment to the return or to each other instead of placing them loosely in the envelope.
  • Treating an extension to file as an extension to pay.
  • Forgetting to mark the name or address change box when that information changed since last year.
