What you'll need
Have your federal Form 1041 available. The Illinois return uses amounts from its Lines 23, 15b, and 21. You will also need the fiduciary's legal name, mailing address, FEIN, and accounting method.
Depending on the entries you make, the form calls for these attachments:
- Nonresident filing: Illinois Schedule NR.
- Income adjustments: Form IL-4562, Schedule 80/20, Schedule M for businesses, Schedule F, Schedule 1299-B, or Schedules K-1-P or K-1-T, as applicable.

Common mistakes
- Skipping Schedule NR as a nonresident. The form tells nonresidents to stop after Step 3 and complete Schedule NR before continuing.
- Completing Schedule D out of order. Read the Schedule D instructions and complete Schedules K-1-T and K-1-T(3) first. Complete Section B with all required beneficiary information before Section A. The form warns that not following these directions may delay processing or lead to requests for more information.
- Leaving out extra beneficiary pages. If you have more beneficiaries than the spaces provided, attach additional copies of Schedule D, Section B. Attach all Section B pages behind Section A.
- Forgetting to show the payment on Page 1. The form specifically reminds you to enter the payment amount at the top of that page.
- Omitting required information. The form warns that failure to provide required information could result in a penalty.
