N-200V: Individual Income Tax Payment Voucher (2025)

Hawaii · Hawaii DoTax · Used for tax years 2025

Hawaii Form N-200V is an individual income tax payment voucher for estimated tax payments, extension payments, and payments for Forms N-11, N-15, or N-310 made separately from the return. It identifies the taxpayer, tax year, payment type, and payment amount.

Who files this form

Use Form N-200V to send the Hawaii Department of Taxation:

  • An estimated tax payment.
  • An extension payment.
  • A tax return payment for Form N-11, N-15, or N-310 for an individual taxpayer, when the payment is not being sent with the return.

For example, use this voucher if you filed electronically but are sending your payment by mail.

What you'll need

To complete and mail the voucher, have:

  • Form N-200V, printed in color, not a photocopy.
  • Your name and Social Security number, or your IRS-issued ITIN if applicable.
  • Your tax year ending date and payment amount.
  • A check or money order in U.S. dollars payable to “Hawaii State Tax Collector.”

Your name must appear on the check or money order. Also write your Social Security number, daytime phone number, payment year, and tax return form number on it, such as “2025 Form N-11.” Composite taxpayers use the entity’s federal employer identification number instead of a Social Security number.

  • Writing or stapling in the area marked “DO NOT WRITE OR STAPLE IN THIS SPACE.”
  • Sending cash or a postdated check. Neither is allowed.
