What you'll need
Have the following ready to complete the 2025 form:
- Your DOH Tax Credit Voucher applicable to the 2025 income year, including its number and credit amount.
- Your eligible taxpayer name, Connecticut Tax Registration Number, and income-year beginning and ending dates.
- For Part II, the credit amounts earned, previously applied, and carried back for each corresponding year listed on the form.
- Form CT-1120K, where the form directs you to also enter the credit amount in Part I-A, Column A.
Common mistakes
- Using the wrong year's form. The form is year specific, and the correct year's form must be used.
- Carrying back credit before applying it to 2025. The instructions for Part II, Line 6, Column C say available credit must first be applied against the 2025 income-year liability. The amount in Column C must not exceed Column A minus Column D.
- Overstating the remaining 2025 credit. Part II, Line 6, Column E must show only the 2025 credit remaining after applying credits to the 2025 income year and claiming any carrybacks.
