Colorado · Colorado DOR · Used for tax years 2025
Colorado Form DR 0104CH is the 2025 Voluntary Contributions Schedule used to donate all or part of your tax refund to selected Colorado charities. It records your designated donations, total contributions, and net refund after contributions.
Who files this form
For tax year 2025, use DR 0104CH to donate all or part of your refund to the listed Colorado charities or funds. If you are making a voluntary contribution and filing on paper, you must submit this schedule with your return.
You cannot complete this schedule unless line 42 of DR 0104 or line 10 of DR 0104EZ is greater than zero.
What you'll need
Have the following ready:
- Your DR 0104 or DR 0104EZ, including the amount on line 42 of DR 0104 or line 10 of DR 0104EZ.
- Your name and Social Security number (SSN) or Individual Taxpayer Identification Number (ITIN).
- The amounts you want to donate to your selected organizations.
- If using line 17, Donate to a Colorado Nonprofit Fund, the registered charitable organization's name and 11-digit Secretary of State registration number.
The back of the 104 Book has more information about the listed charitable organizations.
How to file
Submit DR 0104CH with your DR 0104 or DR 0104EZ so your selected organizations receive the donations you designate. The form specifically requires paper filers making a voluntary contribution to include this schedule with their return.
To complete the schedule:
- Enter your name and SSN or ITIN. Use matching information on page 2.
- Confirm that line 42 of DR 0104 or line 10 of DR 0104EZ is greater than zero. Otherwise, stop.
- Enter your donation amounts on the appropriate lines. For line 17, also enter the charitable organization's name and 11-digit Secretary of State registration number.
- Add lines 1 through 17 and enter the total on line 18. This total cannot exceed the refund amount on the applicable return line.
- Subtract line 18 from that refund amount and enter the result on line 19 as your net refund.
The form does not provide a mailing address or e-filing instructions.
Common mistakes
- Completing the schedule without a positive refund amount. Stop if line 42 of DR 0104 or line 10 of DR 0104EZ is not greater than zero.
- Donating more than your refund. Line 18 cannot exceed the amount on the applicable return line.
- Leaving out the schedule. Submit it with DR 0104 or DR 0104EZ to ensure your selected organizations receive your designated donations.
- Using different identifying information on page 2. Your name and SSN or ITIN must match page 1.
- Expecting to change a donation later. Donation amounts cannot be changed after you submit your return, including through an amended return.
- Assuming the full requested donation will always be paid. Donations may be reduced if your refund is reduced after processing. They will also be reduced if you owe back taxes or a balance to another Colorado government agency or the IRS.
