Colorado · Colorado DOR · Used for tax years 2024
Colorado Form DR 0104CH is the 2024 Voluntary Contributions Schedule for donating all or part of your tax refund to selected charities. It records your designated donations, total contributions, and remaining net refund.
Who files this form
Use the 2024 DR 0104CH to donate all or part of your tax refund to selected Colorado charities. If you make a voluntary contribution and file by paper, you must submit this schedule with your return.
You cannot complete this schedule unless line 39 of DR 0104 or line 9 of DR 0104EZ is greater than zero.
What you'll need
- Your DR 0104 or DR 0104EZ, including the amount on line 39 or line 9, respectively.
- Your name and SSN or ITIN. The name on page 2 must match the taxpayer name on page 1.
- If donating through line 16, Donate to a Colorado Nonprofit Fund, the registered charitable organization's name and 11-digit Secretary of State registration number.
How to file
For paper filing, submit DR 0104CH with your DR 0104 or DR 0104EZ so your selected organizations receive the donations you designate.
- Enter the amounts you want to donate beside your selected funds on lines 1 through 16.
- If you use line 16, enter the registered charitable organization's name and 11-digit Secretary of State registration number.
- Add lines 1 through 16 and enter the total on line 17. This total cannot exceed line 39 of DR 0104 or line 9 of DR 0104EZ.
- Subtract line 17 from that refund amount and enter the result on line 18 as your net refund.
Return the full page. Do not cut it.
Common mistakes
- Completing the schedule when line 39 of DR 0104 or line 9 of DR 0104EZ is zero or less. The form says to stop in that case.
- Entering a total donation on line 17 that exceeds the refund amount on the applicable return line.
- Leaving out this schedule when making a voluntary contribution on a paper return.
- Expecting to change donation amounts after submitting your return. Amounts cannot be changed, including through an amended return.
- Cutting the page instead of returning the full page.
