3816: Electronic Funds Transfer Election to Discontinue or Waiver Request

California Form FTB 3816 lets an entity elect to discontinue electronic funds transfer (EFT) payments or request a waiver of EFT requirements. It records the entity’s information, the reason for the request, and a representative officer’s certification.
