120/165V: 165V Arizona Corporate or Partnership Income Tax Payment Voucher (2024)

Arizona · Arizona DOR · Used for tax years 2024

Arizona Form 120/165V is a payment voucher for corporations and partnerships that electronically filed a 2024 income tax return and are mailing a payment. It identifies the business, the type of return filed, and the amount enclosed for taxes or self-assessed penalties due.

Who files this form

Use Form 120/165V only if you electronically filed a 2024 Arizona Form 120, 120A, 120S or 165 and are mailing a payment for taxes or self-assessed penalties due.

  • Corporations with a tax liability of $500 or more in tax year 2024 must pay their tax liability by Electronic Funds Transfer (EFT).
  • Partnerships that elected to pay tax at the entity level and had a tax liability of $500 or more in tax year 2024 must also pay their tax liability by EFT.
  • Partnerships that did not elect to pay tax at the entity level and self-assessed the information return penalty may use this voucher to submit their payment.

For corporations and partnerships subject to the EFT requirement, a non-electronic payment results in a penalty of 5% of that payment.

What you'll need

To complete the voucher, have these details ready:

  • The corporation or partnership's name, mailing address and required Employer Identification Number (EIN).
  • The name, title and daytime phone number, including area code, of the corporate officer or partner.
  • Whether the payment is for calendar year 2024 or a fiscal year beginning in 2024, with the fiscal year's beginning and ending dates.
  • The type of return filed and, for Form 165 or 120S, whether the entity elected to pay tax at the entity level.
  • The amount of payment enclosed.

Include a check or money order payable to Arizona Department of Revenue. Write your EIN and “2024 Tax” on the payment.

How to file

Use this voucher only for a mailed payment after electronically filing a 2024 Form 120, 120A, 120S or 165. If the form's EFT requirement applies to you, pay your tax liability by EFT.

For a mailed payment:

  1. Complete the business, tax-year and contact information.
  2. Check the type of return filed, including the applicable entity-level tax election option for Form 165 or 120S.
  3. Enter the amount of payment enclosed.
  4. Make your check or money order payable to Arizona Department of Revenue. Write your EIN and “2024 Tax” on it. Do not send cash.
  5. Include the payment with the voucher and mail it to:

Common mistakes

  • Using the voucher without electronically filing an eligible return. Use it only if you electronically filed a 2024 Form 120, 120A, 120S or 165 and are mailing taxes or self-assessed penalties due.
  • Mailing a tax payment when EFT is required. The form requires EFT for corporations and entity-level-tax-electing partnerships with tax liability of $500 or more in tax year 2024. The penalty for a non-electronic payment is 5% of that payment.
  • Sending cash or leaving payment details incomplete. Use a check or money order payable to Arizona Department of Revenue, write your EIN and “2024 Tax” on it, and include it with the voucher.
  • Not paying the full tax by the return's due date. The form warns that full payment is required by that date to avoid interest and penalties.
  • Marking the revenue-use-only area. The form says not to mark this area.

Questions people ask

Can I use this voucher if I filed a paper return?

No. The form says to use it only if you electronically filed a 2024 Form 120, 120A, 120S or 165 and are mailing a payment for taxes or self-assessed penalties due.

Can a partnership use this voucher to pay an information return penalty?

Yes. A partnership that did not elect to pay tax at the entity level and self-assessed the information return penalty may use the voucher to submit payment, provided it meets the electronic-filing condition.

What should I write on my check or money order?

Make it payable to Arizona Department of Revenue and write your EIN and “2024 Tax” on it. Include the payment with the voucher.

What happens if I mail a payment when EFT is required?

Corporations and partnerships that elected entity-level taxation must pay their tax liability by EFT if their tax liability was $500 or more in tax year 2024. Failure to do so results in a penalty of 5% of the non-electronic payment.

Will I receive another notice from the department?

The form says you will not receive an additional notice from the Arizona Department of Revenue unless an error exists on your tax return.
