The 2025 Schedule DS & HOF is for use with Alabama Form 40 or Form 40NR. Schedule DS lists the dependents you claim. Schedule HOF collects information about your dependent or qualifying person for Head of Family.
If you checked filing status 3, Married filing separate, you may claim only dependents for whom you separately provided more than 50% of the total support.
What you'll need
The form asks for the following information, but does not list specific supporting documents:
- Names as shown on your tax return, your Social Security number, and your spouse’s Social Security number.
- Each dependent’s first and last name, Social Security number, relationship to you, and whether you provided more than half of their support.
- For Schedule HOF, the dependent or qualifying person’s name, Social Security number, and relationship to you.
- For Schedule HOF, whether you rent or own the home, whether the dependent or qualifying person lived with you, and whether you paid more than 50% of their support.
- For Schedule HOF, whether you are married, divorced, or legally separated, and the applicable dates.
Common mistakes
- Do not include yourself or your spouse as a dependent on Schedule DS.
- If you checked Married filing separate, do not claim a dependent unless you separately provided more than 50% of that dependent’s total support.
- Enter your total number of dependents on Schedule DS, line 1b, and on Form 40, Page 2, Part III, line 1, or Form 40NR, Page 2, Part V, line 1.
- On Schedule HOF, do not repeat names and Social Security numbers in the tax-return identification area if they are already shown on the other side.
