What you'll need
The form asks for the following information:
- Your name as shown on your tax return, your Social Security number, and your spouse’s Social Security number.
- For each dependent: first and last name, Social Security number, relationship to you, and whether you provided more than half of their support.
- For Schedule HOF: the dependent or qualifying person’s name, Social Security number, and relationship to you; whether you rent or own the home maintained for them; whether they lived with you; and whether you paid more than 50% of their support.
- Schedule HOF also asks whether you are married, divorced, or legally separated, and requests the applicable dates if you answer yes.
The form does not list supporting documents to attach.
Common mistakes
- Do not include yourself or your spouse in the dependent list on line 1a.
- If you checked filing status 3, Married filing separate, claim only dependents for whom you separately provided more than 50% of total support.
- On Schedule HOF, do not repeat the tax-return name and Social Security number information if it is shown on the other side.
