FDT-V: Fiduciary Income Tax Payment Voucher (Revision)
Alabama · Alabama DOR · Used for tax years 2025
Form FDT-V is Alabama’s Fiduciary Income Tax Payment Voucher for payments associated with Form 41. It identifies the taxpayer, fiduciary, tax period, payment type, and amount paid.
Who files this form
Taxpayers making a payment for taxes due on an Alabama Form 41 return, or making a voluntary estimated tax payment, must complete and file Form FDT-V unless they pay electronically.
Do not submit Form FDT-V if no payment is due.
What you'll need
Have the following information ready to complete the voucher:
- The taxpayer’s legal name and complete mailing address.
- The fiduciary’s name and title.
- The entity’s Federal Employer Identification Number (FEIN).
- The last day of the taxpayer’s taxable year.
- The payment amount and payment type.
If mailing the voucher, include your full payment. If paying through My Alabama Taxes (MAT), you need a Sign On ID and Access Code to log in.
How to file
Enter the taxpayer and fiduciary information, FEIN, payment amount, and tax period. For the tax period, use the last day of the taxpayer’s taxable year. Check only one payment-type box: Return, Amended, Estimate, or Extension Payment.
By mail: Detach the voucher along the marked line and mail it with your full payment. The address printed for payments with Form 41 or estimated payments is:
Alabama Department of Revenue
Income Tax Administration Division
PO Box 327444
Montgomery, AL 36132-7444
Place the payment, voucher, and any return loose in the envelope. Do not staple or otherwise attach them to one another.
Electronically: Pay by direct debit through an approved e-file software vendor or through My Alabama Taxes (MAT) at www.myalabamataxes.alabama.gov. MAT requires a Sign On ID and Access Code. Do not file Form FDT-V when paying electronically.
Common mistakes
- Submitting a voucher when no payment is due. The form says not to submit Form FDT-V in that situation.
- Checking more than one payment-type box. Check only one box.
- Stapling or attaching the items together. Keep the payment, voucher, and return loose in the envelope.
- Treating a filing extension as extra time to pay. Full payment is due by the return’s original due date, without regard to any filing extension.
