FDT-V: Fiduciary Income Tax Payment Voucher (Revision)
Alabama · Alabama DOR · Used for tax years 2024
Form FDT-V is Alabama’s Fiduciary Income Tax Payment Voucher for Form 41 payments. It identifies the taxpayer and fiduciary, tax period, payment type, and amount paid.
Who files this form
Taxpayers making a payment for taxes due on an Alabama Form 41 return, or making an estimated tax payment for that return, must complete and file Form FDT-V unless they pay electronically.
Do not submit Form FDT-V if no payment is due.
What you'll need
Have the following information ready to complete the voucher:
- The last day of the taxpayer’s taxable year.
- The entity’s Federal Employer Identification Number (FEIN).
- The taxpayer’s legal name and complete mailing address.
- The fiduciary’s name and title.
- The payment type and amount being paid.
Mail the voucher with your full payment.
How to file
Enter the taxpayer and fiduciary information, FEIN, and payment amount. For the tax period, enter the last day of the taxpayer’s taxable year. Mark only one payment type box: Return, Amended, Estimate, or Extension Payment.
Detach the voucher along the marked line. For payments with Form 41 or estimated payments, mail the voucher and full payment to:
Alabama Department of Revenue
Income Tax Administration Division
PO Box 327444
Montgomery, AL 36132-7444
Place the items loose in the envelope. Do not staple or otherwise attach the payment or voucher to your return or to each other.
You may instead pay electronically by direct debit through approved e-file software vendors or through My Alabama Taxes (MAT) at www.myalabamataxes.alabama.gov. MAT requires a Sign On ID and Access Code. You do not need to file Form FDT-V when paying electronically.
Common mistakes
- Checking more than one payment type box. Check only one.
- Stapling or attaching the payment, voucher, or return together. Place the items loose in the envelope.
- Submitting Form FDT-V when no payment is due. Do not submit it in that situation.
- Treating a filing extension as extra time to pay. Full payment is due by the return’s original due date, regardless of filing extensions.
